This is a registration form that will help us better plan for, and schedule playing times for, our adult futsal players for 2021. The purpose of the form is to let us know your AVAILABILITY during the week.  Thank you for helping us plan better by completing this form!

Games will be played every Tuesday night, from 7-10 P.M. in the gym at the Shepherd's Church. Games will start April 13th and end on June 29th.
