This registration form is only for those wishing to register their child for the evening session of our summer soccer camp for the week of June 21st-24th. The camp will run from 5:00-8:00 PM for these four evenings. It might be helpful to show up a few minutes earlier for check in on the first day (Monday). Cost for the camp is $95.

I agree that The Shepherd's Church may use such photographs of me and my child with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
